Medicaid care plans and case managers in Tampa Bay

Once someone is approved for Long-Term Care, a private health plan runs their services and a case manager writes their plan of care. Here is who those plans are in our three counties.

The short answer

Pasco and Pinellas are in Medicaid Region C, and Hillsborough is in Region D. Under the 2025 to 2030 state contracts, Florida Community Care, Humana, Simply Healthcare and Sunshine Health offer long-term care in both regions. Aetna Better Health and UnitedHealthcare also serve Region D. Your plan's case manager authorizes services, so that is the person to ask for home care hours.

What your case manager does

Care coordination and case management is a covered Long-Term Care service. Your case manager assesses needs, builds the plan of care with you, authorizes services and checks in. If you ask about the Participant Direction Option, the plan must assign a case manager trained in it within two business days of your choosing it.

How to get care from Aster Care under a plan

  1. Tell your case manager what is hard. Be specific about mornings, bathing, meals, nights and falls. Hours follow documented need.
  2. Ask for the agency you want. Plans use provider networks, so ask whether the agency you prefer is in network.
  3. Call us with your plan's name. We will tell you whether we can take the authorization.

A plan is not the only option

People on the iBudget waiver are not in these plans. AHCA says iBudget is fee for service and is not offered by health plans. See the iBudget guide.

About this page. Aster Care LLC is a private home care agency, not a government agency. Eligibility decisions are made by the state offices named here. This is general information, not legal or benefits advice, and program rules change, so please confirm details with those offices.
